Quality Improvement QI ; teams were established and a half day orientation was given for health providers and the youth who are members of the team representing the facilities that have integrated YFS. The QIT are identified and selected from each health facility in Addis Ababa and intervention sites in Oromiya with a total of 152 from 38 facilities. Management of Separation Anxiety with Clomipramine Clomicalmm ; Clomipramine is the only drug that is been either registered worldwide or shown in well-controlled published studies to be effective in separation anxiety in dogs. Although a preliminary study found no clear effect in dogs with "separation problems" [9], four well-controlled studies have demonstrated clear advantages of adding clomipramine to behaviour therapy in dogs with separation anxiety [6, 7, 27, 28]. The benefits of clomipramine were to produce a much faster control of signs of destruction, defecation and urination, and the advantages were maintained throughout the 2 - 3 month trial example of results in Fig. 1, [6] ; . Following the presentation, CVM, the Acting Commissioner and FDA Legal Counsel agreed to recall ProHeart 6 from the market. After two days of negotiating with the FDA, Wyeth voluntarily recalled ProHeart 6 from the market on September 4, 2004. Shortly after the recall of ProHeart 6, Wyeth sought a review of the recall decision through a meeting of the Veterinary Medicine Advisory Committee VMAC ; . The FDA granted the request for a VMAC meeting and scheduled it for January 2005. It appears the timing of the VMAC would have allowed Wyeth a chance to reintroduce ProHeart 6 for the spring heartworm season if the VMAC voted to support its return to the market. In preparation for the VMAC meeting, Dr. Hampshire prepared a presentation regarding the thousands of ADEs received and worked to ensure that the advisory committee would have complete information regarding these events. Documents obtained and reviewed by the Committee, coupled with interviews conducted by Committee staff, appear to support allegations that Wyeth investigated Dr. Hampshire and presented its findings to Dr. Crawford. Following Wyeth's presentation, Dr. Hampshire was removed from the review of ProHeart 6 and subjected to a criminal investigation by the FDA. FDA Investigators advised Committee staff that the criminal investigation resulted in no action taken against Dr. Hampshire. Furthermore, the FDA recently gave Dr. Hampshire an award for her job performance related to ProHeart 6. Information available to the Committee appears to support allegations that Wyeth's efforts to discredit Dr. Hampshire were not limited to the FDA. Attached is a two-page letter from a veterinarian and former commissioned officer in the United States Public Health Service. According to the letter, a Wyeth sales representative in Alabama stated that Dr. Victoria Hampshire was the sole reason for the recall of ProHeart 6. Further, the Wyeth representative stated that Wyeth investigated Dr. Hampshire and said that she pursued the withdrawal of ProHeart 6 for personal financial gain. Finally, the Wyeth representative added that once "[Dr. Hampshire] was taken care of" the number of adverse event reports being submitted for ProHeart 6 dropped significantly. Subjects All patients were referred by neurologists and screened by the neurosurgeon CT ; . Inclusion criteria were liberal. Candidates for surgery required a history compatible with idiopathic PD, responsiveness to levodopa, or at least a history of such, a Hoehn and Yahr 1967 ; severity score of 3 or greater while in the "on" condition, and clinical symptoms in addition to tremor and rigidity. The only absolute contraindications for surgery were dementia more than mild ; and Parkinson's Plus syndromes. Surgical exclusion criteria included other severe medical illnesses, bleeding diatheses and a head too large for the stereotactic frame. There were 23 PD patients studied, 18 males and 5 females, all Caucasian, with a mean age of 61 9 S.D. ; years of age. However, based on these data, it is possible that AIbZIP expression is restricted to hormone-sensitive tissues, i.e., the prostate and mammary gland. Androgen Regulation of AIbZIP Expression in LNCaP Cells. To additionally characterize the regulation of AIbZIP expression by androgens we assessed the effect of R1881 on AIbZIP mRNA levels as a function of time and dose. AIbZIP mRNA levels were determined in LNCaP cell cultures between 1 and 72 h after the addition of 0.1 nM of R1881. This dose of R1881 was selected because it is the same dose that was used to treat LNCaP cultures that served for the cDNA subtraction. As shown in Fig. 7A, AIbZIP mRNA levels expressed as AIbZIP: -actin ratios ; did not change markedly up to 12 after the addition of 0.1 nM of R1881. However, 23-fold changes in AIbZIP mRNA levels were observed starting 24 h after the addition of R1881. Similar results were observed in two other experiments. The induction of AIbZIP expression by R1881 was less rapid than that of other androgen-responsive genes such as NKX3.1 17 ; and PSA 28 ; , which suggests that AIbZIP is not an early response gene. Therefore, we examined the effect of the protein synthesis inhibitor cycloheximide and the RNA synthesis inhibitor actinomycin D on R1881-induced AIbZIP expression in LNCaP cells. Promising new therapies, including the pegylated interferons, should also be studied in children. Some patients may benefit from treatment even if the liver disease is mild. Given the long life expectancy of children and their better tolerance to drugs, the long-term safety of these medications needs to be studied in children.
MALDI-TOF MS was used to identify the presence of the BHMT enzyme. This enzyme catalyzes the first step in a three-enzyme pathway that promotes the methylation of PE to PC, a major biliary excretory product. Immunoblotting of canalicular membranes confirmed the presence of BHMT along with PEMT, an enzyme that catalyzes the third step in the conversion of PE to PC, and immunofluorescent microscopy confirmed the localization of PEMT to the canalicular membrane. Immunoblotting also revealed the presence of SR-BI in canalicular membranes, and these findings were confirmed by immunofluorescent microscopy. These findings indicate that the PE methylation pathway operates at or in close proximity to the canalicular membrane and possibly contributes to phospholipid and cholesterol excretion into bile. A schematic of this pathway is provided in Fig. 7. The flux of phospholipid through the liver each day is large and, in the mouse, is estimated to approximate the entire liver phospholipid pool 17 ; . This phospholipid pool is derived largely from de novo synthesis by two pathways. In one pathway present in all nucleated cells, CDPcholine and diacylglycerol react to form PC catalyzed by CDP-choline: diacylglycerol phosphocholinetransferase. In the PE methylation pathway, which is quantitatively significant only in liver, betaine transfers a methyl group to homocysteine via BHMT to form methionine. Methionine then transfers a methyl group to adenosine via MAT to form S-adenosylmethionine. Finally, three methyl groups are transferred from S-adenosylmethionine to PE to form PC. Under normal metabolic conditions, it is thought that the CDP-choline pathway accounts for 6080% of liver PC biosynthesis 18 ; . The CDP-choline and the PE methylation pathways can both provide PC for lipoprotein and membrane biosynthesis and bile secretion 19 ; . In the current study, we show that BHMT and PEMT also localize to basolateral and canalicular membranes, suggesting they may serve to convert PE, known to be present in relatively high concentrations in the inner leaflet of plasma membranes, to PC for specific needs 20, 21 ; .
